Intake Appointment & Subsequent Sessions

The initial intake appointment will take about 60 minutes and will include an evaluation to assess your current symptoms/presenting problem and is an opportunity for both parties to determine fit.  Subsequent sessions are generally 45-55 minutes, either weekly or biweekly.

Cancellation Policy

Windy City Therapy requires a 24 hour notification for cancelled appointments. A cancellation fee will be incurred for all missed appointments when a 24 hour notification has not been provided.

Emergencies

In an emergency please dial 911 or go to your nearest emergency room.  To reach the Suicide Prevention Hotline call 1-800-273-TALK (8255) or text HOME to 741741.

Confidentiality

Confidentiality is extremely important within the therapeutic relationship and is maintained at all times. Everything we discuss will remain between us and you can rest assured you have a safe space to explore your thoughts and feelings.  There are some instances however, where confidentiality must be broken:

  1. If I believe that you are likely to harm yourself and/or another person, I may take action necessary to protect you or others by contacting law enforcement officers or a physician.

  2. If I have cause to believe that a child has been or may be abused or neglected, I am required to make a report to the appropriate state agency.

  3. If I have cause to believe that an elderly or disabled person has been or may be abused, neglected, or subject to financial exploitation, I am required to make a report to the appropriate state agency.

  4. If your records are requested by a valid subpoena or court order, I must respond.
